Imperial Motus Med embarked on a new process to manage beneficiaries with multiple diseases, where previously members had to register with any one or more disease management programmes.

The managed care company now identifies the high-risk beneficiaries from claims and health assessment data received. Based on this data beneficiaries are categorised in low, medium and high risk. Based on this categorisation specialist care coaches are assigned to the identified high-risk beneficiaries. They will assist the beneficiaries with understanding their risks and supply information to adequately manage the risks.

In addition, they will identify and apply additional benefits and protocols to effectively manage their patients.

The Scheme offers the following three programmes for the management of HIV, oncology and maternity in addition to the above-mentioned process:

  • HIV YourLife Programme
  • Oncology Benefit Management Programme
  • Maternity Programme

Imperial Motus Med has partnered with Momentum Health Solutions, the Scheme's healthcare administrator, to provide our valued members with access to a Palliative Care Programme. Palliative care is specialised medical care for people living with a serious illness and is focused on providing relief from the symptoms and stress of their condition(s).
