Who can be dependants?

- A copy of the marriage certificate

Grandchild
Grandchildren are allowed if:
The parent of the grandchild is still dependent on the principal member and on Imperial Motus Med.
If the above does not apply, the application is subject to Scheme approval and legal documentation is required indicating that the grandchild was legally adopted or placed in the care or custody of the principal member. - ID or birth certificate, affidavit

Proof of registration at educational institution

Annual proof needs to be provided to the Scheme on whether a child over the age of 25 is still financially dependant on the member.

Indigent parent/siblings
The parent or sibling must be financially dependent on the member, who should be liable for their support. Indigent parents or siblings will be charged adult dependant contribution rates.

Removing a dependant 30-days’ notice is required to remove/terminate a dependant's membership.

Membership

Resignation A member can only resign from Imperial Motus Med if he/she resigns from the Imperial Limited and Motus Holdings Limited or joins his/her spouse's or partner's medical aid as a dependant.

Retirement Members of Imperial Motus Med who retire or whose employment is ended by the employer on account of age, ill health or a disability, may choose to retain their membership. Please note that the full contribution will be your responsibility and the company subsidy may be discontinued. Your monthly contributions will be due in advance on the 1st of every month for that month.

Continuation of membership of registered dependants after principal member's death The dependants of a deceased member, who are registered with the Scheme as his/her dependants at the time of death, are entitled to and may choose to retain membership of the Scheme until they become members of another medical scheme.

Suspension
Failure to pay contributions will give the Scheme the right to suspend all benefit payments in respect of claims which arose during the period of default. Written notice of debt will be given to the member/employer, failing which membership may terminated.

Termination
Failure to pay contributions
Membership can be terminated due to the non-payment of contributions.
Fraudulent activity

The Board of Trustees may exclude benefits or terminate membership of any member or dependant in the event of:
- abuse of privileges;
- fraud;
- non-disclosure of material information;
- submission of false claims;
- misrepresentation.
